Baby with fever / very unwell child

What to do

  1. Hard to wake, breathing difficulty, blue lips or first seizure: emergency help now.
  2. Stiff neck or rash that does not fade under pressure: emergency help now.
  3. Grunting or chest pulling in, grey/pale skin or lips, abnormal response or light sensitivity: emergency help now.
  4. Under 3 months: 38 °C or higher. At 3–6 months: 39 °C or higher. Urgent assessment, also for suspected fever.
  5. Use a digital armpit thermometer as instructed. Do not delay help.
  6. Awake and swallowing safely: offer fluids or breastfeeding. Check regularly, including at night.
  7. Unresponsive with abnormal breathing: follow the dispatcher and open child CPR.
  8. Urgent advice for refusing feeds, unusual behaviour, fewer wet nappies or fever lasting 5 days. Seek advice if worried even below those temperatures.

What to avoid

  1. No sponge cooling or excessive wrapping. No aspirin or improvised doses.
